Rates
Currently my 50 minute hourly fee is 145.00. If you have a demonstrable financial need we can discuss a lower rate. I am a provider for Anthem BCBS and IU Health. They have negotiated my rate down from my posted rate, so if you have either one of these insurance companies, I will take that negotiated rate out of pocket until your deductible is met. We can discuss this further when we meet.
If you are paying out of pocket, I will collect via HSA or credit card through my Electronic Health Record software. I file your insurance claims as a courtesy. You are responsible for knowing your insurance coverage. You are ultimately responsible for meeting all deductibles, copays, and charges that your insurance carrier does not cover, including administrative services, missed session fees, and paperwork requested by you from my office. Fees are subject to change for these services.
Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- What is the coverage amount per therapy session?
- Is pre-approval required before starting therapy?
Cancellation Policy
If you miss your scheduled appointment with less than 48 hours advance notice, you will be required to pay the fee that I would have collected if you had made your appointment. Any change in session scheduling that interferes with submitting your session to your insurance company will be your financial responsibility. If you are a private pay client, I will charge you the agreed upon session rate if you cancel with less than 48 hours notice. Cancellation exceptions will rarely be made, and are at the discretion of the therapist. Illness of self or child is not an automatic exception to my 48 hour policy. Please do not come to sessions with an active illness. I reserve the right to refuse service if this occurs and charge you for a missed session.
Reduced Fee
Reduced fee services are available on a time-limited basis.
Payment
Cash, check and all major credit cards accepted for payment, but preferred methods are checks or cash. I will include the surcharge for using a major credit card.
